JOB SUMMARY:

 

The Transition Specialist will work toward the development and implementation of transition services and planning for students with disabilities - ages 16 years and older. Collaboration between school, students, parents, community business leaders, tribal entities, state programs, federal programs and other adult service providers will be required for the development of this transition system. Other types of curriculum programs will also be utilized.

 

DUTIES and RESPONSIBILITIES:

 

  1. Instructs classes in Life Skills and Career Exploration for students in special education as well as students in general education who may benefit from such courses.
  2. Work with small student groups for intensive basic skills recovery and development. May use reading, writing, and math curriculum programs.
  3. Attend all IEP meetings for students who will turn 16 or older to ensure compliance with all transition requirements.
  4. Promote/coordinate programs to support student self-advocacy and ensure that each student and their parent(s) or guardian understand the transfer rights from parents to student.
  5. Administer a variety of age-appropriate transition assessments.
  6. Arranges transition skill activities and events for students throughout the school year.
  7. Works cooperatively with other staff for the planning and implementation of inclusive practices.
  8. Implements age and ability appropriate corrective measures, disciplines as needed in a way that is appropriate for management of classroom environment.
  9. Guide student in making decisions about post-secondary choices such as work, training or further education (vocational or higher education).
  10. Arrange placement and provide supervision of students in job experiences; serve as a job coach.
  11. Write transition post-secondary goals and transition activities for student IEPs in collaboration with the Special Education Teachers.
  12. Create and follow the school’s transition plan in transitioning students from Middle School to High School and High School to Post Secondary.
  13. Prepare a Summary of Performance (SOP) form for the student before they graduate, age out or leave the program at the end of their high school term.
  14. Monitor student activities and behavior during all group activities including meals, field trip, recess and other school events.
  15. Administer emergency and/or basic first aid to students, when necessary. Assist students to health aide office.
  16. Assist, when appropriate, with related services providers.
  17. May transport students to primary residence and to service providers along with another staff member as necessary.
  18. Participates in a variety of professional development and improvement programs.
  19. Works with staff for development of annual program goals and objectives.
  20. Attends Professional Development related to transition/IEP regulations.
  21. Establishing, maintaining, and supporting positive and effective working relationships with colleagues, departments, outside agencies, and stakeholders involves clear communication, collaboration, and mutual respect.
  22. Utilizing various communication channels, including social media, can enhance connectivity and engagement, fostering stronger connections and facilitating effective collaboration.
  23. Performs other duties as assigned.

 

REQUIREMENTS:

 

  1. Will support and comply with the Rock Point Community School philosophy, mission, and vision statement.
  2. Will have four (4) hours Prevention of Sexual Harassment training annually.
  3. Will have two (2) hours of SCAN procedures and reporting annually.
  4. Be in compliance and maintain confidentiality according to FERPA (Family Educational Rights & Privacy Act).

 

QUALIFICATIONS:

 

  1. State of Arizona Certification in Education or Special Education with Special Education endorsement.
  2. BA or MA degree, Education/Special Education preferred or Psychology or related field
  3. Have knowledge of Special Education.
  4. Have basic knowledge of Special Education program referral process, evaluation, development, and implementation of Individual Education Programs.
  5. Understanding of inclusive practices principles and reasoning.
  6. Have two or three years experience working with exceptional students who are in middle school or older and have at least 15 hours of graduate level SPED related courses/transition courses.
  7. Conducts duties and responsibilities in accordance with the RPCS Policy and Procedures, and other reporting policies and procedures of the tribe, state, and federal government.
  8. Must have minimal knowledge of Dine language, culture, history, traditions, and its people for compliance in all aspects of educational programs.
  9. Have current CPR & 1st Aide Certification and basic safety training.
  10. Must have a valid government issued driver’s license or identification card.
  11. Must have Federal, Arizona, and Navajo Nation background clearances.
